                   Proposal for Consultant/Broker for Health and Welfare Plans

                    3. Enrollment/re-enrollment  including  on-line  portals  -  Burnham  Benefits  has  extensive
                       experience in employee communication during open enrollment and throughout the year.  At
                       no additional fee we will:
                              Work with Francis Parker School’s team to develop the strategy and expectations for
                              employee meetings
                              Coordinate carrier attendance and delivery of carrier supplies
                              Create open enrollment announcements to highlight the benefit changes and instructions
                              for open enrollment.
                              Create and print custom Benefit Summaries that summarize the company’s benefit plans,
                              contributions, and carrier contacts.
                              Post carrier benefit summaries and required annual notices on online enrollment site.
                              Host employee benefit meetings with a PowerPoint presentation encompassing all the
                              benefits.
                              Conduct face-to-face Open Enrollment meetings at all your locations with the help of a
                              qualified vendor.
                              Create  personalized  benefit  statements  (hidden  paychecks)  to  help  employees
                              understand the true value and expenditure of their benefit plans.
                              Provide various health and wellness pieces to educate employees.
                              Provide handouts pertaining to the topic of the meeting.
                              Provide our 1-800 number employee call-in line for claim assistance.
                              Work with your HR team and the carriers on the timely submission all open enrollment
                              paperwork  (renewal  confirmations,  enrollment  forms,  master  applications  for  new
                              carriers).
