MetLife | Supplemental Benefits
You may purchase individual policies from MetLife. Your premiums are paid through payroll deductions on an after-tax basis.
MetLife policies offer direct-to-the-policyholder cash payouts to help cover what other insurance doesn’t. All of the MetLife indi-
vidual policies are portable, which means that you can keep them should you change jobs or retire, with no increase in premi-
ums.
MetLife | Group Accident
You have the opportunity to apply for Group Accident Insurance through MetLife. Group Accident pays benefits to help you cov-
er the unexpected costs associated with accidental death or injuries. Benefits are flat dollar payments and are not based on med-
ical expenses incurred. You have a choice of two levels of coverage – Low Plan or High Plan. Benefit amounts vary by the plan you
choose. You may also apply for coverage for your spouse/registered domestic partner and your dependent children.
Health Benefit Screening: MetLife will provide an annual benefit of $60 per calendar year for taking one of the eligible
screening/prevention measures. MetLife will pay only one health screening benefit per covered person per calendar year.
MetLife | Critical Illness
You have the opportunity to apply for Critical Illness Insurance, a limited benefit policy offered through MetLife. This insurance is
designed to help you offset the costs of certain covered critical illnesses such as stroke, permanent paralysis, heart attack and
end stage renal disease. When diagnosed with a covered condition, you will receive a lump-sum benefit that you can use for ex-
penses such as deductibles, copays, transportation, and so on.
You may apply for Critical Illness Insurance coverage with either a $10,000 or $20,000 benefit with no medical questionnaires or
exams. You may also apply for coverage for your spouse/registered domestic partner and dependent children. You must be en-
rolled in a major medical plan in order to apply for Critical Illness Insurance. Please refer to the 2018 Monthly Rate Sheet.

MetLife | Hospital Indemnity
You have the opportunity to apply for Hospital Indemnity Insurance through MetLife. Hospital Indemnity pays benefits to help
you cover the unexpected costs associated with hospital admission and confinement due to accident or sickness. Benefits are
flat dollar payments and are not based on medical expenses incurred. You have a choice of two levels of coverage – Low Plan or
High Plan. Benefit amounts vary by the plan you choose. You may also apply for coverage for your spouse/registered domestic
partner and your dependent children.
Eligible Individual Initial Benefit Requirements
Employee $10,000 or $20,000 Coverage is guaranteed provided you are actively at work.
Spouse/Domestic 50% of the employee’s Coverage is guaranteed provided the employee is actively at work and the
Partner Initial Benefit spouse/domestic partner is not subject to a medical restriction as set forth on the
enrollment form and in the Certificate.
Dependent 50% of the employee’s Coverage is guaranteed provided the employee is actively at work and the de-
