         Teladoc
         Teladoc is a convenient and affordable alternative to Emergency Room and Urgent Care visits that gives you ability to resolve many
         common medical issues 24 hours a day, 7 days a week, 365 days of the year by phone, mobile app or online video. You and your
         covered dependents, enrolled in any Alexander’s Mobility Services’ medical plan, have access to this program.

         Contact  Teladoc  from  home,  work,  or  on  the  road  and  let  the  doctor  come  to  you! Teladoc  doctors  diagnose  non-emergency
         medical problems, recommend treatment, and can even call in a prescription to your pharmacy of choice, when necessary.

         Teladoc doctors are U.S. board-certified, licensed in your state and average 15 years  of practice experience.  Teladoc does not
         replace your primary care physician. With your consent, Teladoc will provide information about your consult to your primary care
         physician.

         With Teladoc, you can talk to a doctor 24/7/365 by phone, online video or mobile app. Use Teladoc for medical advice and care
         when:
         •   Your primary care doctor is not open.
         •   You are at home, traveling or do not want to take time off work to see a doctor.
         •   You need a prescriptions or refills

         Cost
         •   EPO and POS members will be responsible for a $0 copay.

                         Registering for Teladoc:
                         You can use Teladoc anywhere you have Internet access. Simply:
                            1. Visit www.MyDrConsult.com and click “Set Up Account.”
                            2. Enter your name, date of birth, ZIP code, email address, preferred language and gender and click
                                “Continue.” The system will identify you based on this information. If you’re unable to be identified,
                                you’ll be directed to Teladoc Customer Service.
                            3. On the next screen, enter the required information and click “Set Up My Account.”
                            4. Complete your profile by clicking on “My Medical History.” You can enter your history right after
                                 registering or you can come back to finish it later.
