The Avesis vision plan provides professional vision care and high quality lenses and frames through a broad network of optical
specialists. You will receive richer benefits if you utilize a network provider. If you utilize a non‐network provider, you will be
responsible to pay all charges at the time of your appointment and will be required to file an itemized claim with Avesis.

Employee Assistance Plan (EAP): This plan provides 24/7 phone counseling services, and
up to 4 face-to-face visits for you and anyone in your household. The EAP counselors
provide counseling for stress, financial stress, work/life matters, and more.

Travel Assistance Plan: This plan includes a wealth of services to assist you when you are
traveling 100 miles or more from home. Services are provided for business and leisure
travel. You can receive 24 hour help on various topics from weather forecasting to
emergency medical help.
