Voluntary Worksite Plans
Voluntary Accident
Transamerica Accident insurance will pay you and your family a benefit to help cover your out-of-pocket medical costs in case of an
off the job accident. For covered accidental injuries, lump sum benefits are paid directly to you, regardless of any other coverage.
Payments are made according to a fixed schedule that includes benefits for hospitalization, fractures and dislocations, emergency
room visits, major diagnostic exams, and physical therapy. Other covered treatments and services include urgent care visits,
medical imaging (MRI, MR, CT, CAT, EEG), burns, surgery and more. For example, if you had an emergency room visit, Transamerica
would pay you $175. You will also receive a $50 wellness benefit paid out each year to you and your spouse for completing annual
preventive screenings. As with all the Transamerica plans, there is no coordination of benefits, and the money you receive is yours
to spend as you choose.
Voluntary Critical Illness
Transamerica Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-
threatening illness. You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as a Heart Attack, Stroke,
Cancer, Kidney Failure and more. You may elect $15,000 or $20,000 for yourself, your spouse, and child(ren). This plan also has a
$100 wellness benefit that pays out once per year per covered person for completing annual preventive screenings (in addition to
the wellness benefit for you and your spouse on the Accident plan if enrolled in both).
Hospital Indemnity
Hospital Indemnity insurance coverage provides you with payments when you are admitted for 24 hours or more, and per day
when you are confined to a hospital, due to a covered accident or illness. Your plan will pay $750 for an admission (24 hours or
more), and a daily amount of $100 per day, up to 31 days. Hospital Indemnity also pays extra benefits for admission to and/or
confinement in an Intensive Care Unit (ICU), and for other benefits and services.
