SUPPLEMENTAL BENEFITS

You may purchase individual policies from MetLife including Group Accident, Group Hospital Indemnity, Home & Auto, and Voluntary
Legal. Your premiums are paid through payroll deductions on an after-tax basis. MetLife policies offer direct-to-the-policyholder cash
payouts to help cover what other insurance doesn’t. All of the MetLife individual policies are portable, which means that you can keep
them should you change jobs or retire, with no increase in premiums.

METLIFE | GROUP ACCIDENT INSURANCE
Group Accident insurance is designed to help you meet the out-of-pocket expenses and extra bills that can follow an accidental injury,
whether minor or catastrophic. Accident lump sum benefits are paid directly to you based on the amount of coverage listed in the schedule
of benefits. The accident base plan is guaranteed issue, so no health questions are required.

METLIFE | HOSPITAL INDEMNITY INSURANCE

Group Hospital Indemnity insurance is designed to help provide financial protection by paying a benefit due to a hospitalization and in
some cases, for treatment received for an accident or sickness, even if that treatment occurs outside the hospital. Indemnity lump sum
benefits are paid directly to you based on the amount of coverage for the service outlined in your summary of benefits, regardless of
the actual cost of treatment.

METLIFE | HOME & AUTO INSURANCE

MetLife Auto & Home can help you with convenient access to a comprehensive range of protection that goes beyond traditional offerings.
Coverages offered include auto (including roadside assistance), homeowners, umbrella liability, renters / condo, dwelling, watercraft,
motorcycle, mobile homes, and recreational vehicle insurance.

Please Note: Home Insurance is not available in the state of Florida.

METLIFE | VOLUNTARY LEGAL SERVICES

MetLife / Hyatt Legal provide you with convenient and affordable access to a highly qualified network of more than 13,500 attorneys for
counsel on the most frequently needed personal legal matters. Additionally, all group legal services include telephone advice and office
consultations on an unlimited number of matters.

Benefits include document review, document preparation, general telephone advice, and office consultations. Legal plans cover a broad
range of legal matters, which may include family & personal, money matters, vehicle & driving, home & real estate matters, civil lawsuits,
wills, estate & future planning, and elder-care. There are no hour limits, no page limits, no co-pays and no waiting periods when using
attorney network services. When you use a network attorney for covered services, all attorney fees are paid by the plan. Plan members may
also receive service through any attorney, anywhere, anytime. When using an out-of-network attorney, you will be reimbursed according
to a set fee schedule.
