Disability Insurance







         Unum | Long Term Disability Insurance
         American  Business  Bank  offers  you  Long  Term  Disability  (LTD)  to  provide  income  replacement  if  you  become  disabled  for  an
         extended period of time, through Unum. If you become totally and permanently disabled, benefits begin 360 days after the start of
         your illness or injury and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD works with
         state disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit
         equal to 60% of your pre‐disability earnings up to a maximum benefit of $12,500 per month. Pre-existing condition exclusions
         apply. See contract for details.



                     Long Term Care Insurance






         Unum | Long Term Care Insurance
         American Business Bank provides you with the option to purchase Long Term Care insurance. Long Term Care is received in the
         home or a facility when you need help with 2 or more activities of daily living (bathing, eating, dressing, transferring, continence) or
         if you suffer severe cognitive impairment (Alzheimer's). Newly eligible employees will not be subject to EOI and level premium
         locks in your age at the initial enrollment.

         You can structure your plan by choosing selecting the coverage that is right for you. Benefit options range from $2,000 to $9,000
         with benefit durations of 3 years to lifetime. If you enroll within your initial eligibility period, you can elect up to a $6,000 monthly
         benefit without answering any medical questions.

         Plan coverage options include:
         •   Plan 1: Nursing Facility, Home and Community Based Care without Inflation
         •   Plan 2: Nursing Facility, Home and Community Based Care with Simple Inflation
         •   Plan 3: Nursing Facility, Home and Community Based Care with Compound Inflation
