         Unum | Critical Illness

         Unum Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-threatening
         illness.  You’ll  receive  a  lump-sum  cash  payment  upon  diagnosis  of  a  covered  condition  such  as  a  Heart  Attack,  Stroke,  Kidney
         Failure, and Cancer.

         Unum | Accident
         Unum Accident insurance will pay you a benefit to help cover your out-of-pocket medical costs in case of an accident. For covered
         accidental injuries, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid according to a fixed
         schedule  that  includes  benefits  for  hospitalization,  fractures  and  dislocations,  emergency  room  visits,  major  diagnostic  exams,
         physical therapy, and more.

         Unum | Whole Life
         Whole Life Insurance provides you with “living benefits” you can use when you need them, as well as a death benefit.

         Unum | Hospital Indemnity
         Group Hospital Indemnity insurance is designed to help provide financial protection for covered individuals by paying a benefit due
         to a hospitalization and in some cases, for treatment received for an accident or sickness, even if that treatment occurs outside the
         hospital. Employees can use the benefit to meet the out-of-pocket expenses and extra bills that can occur. Indemnity lump sum
         benefits are paid directly to the employee based on the amount of coverage listed, regardless of the actual cost of treatment. A
         health screening benefit is paid for the completion of specified routine wellness screenings such as mammography, stress test,
         colonoscopy, chest x-ray, and many more.

         Wamberg Genomic Advisors (WGA) | Cancer Support
         This  plan  provides  personalized  support,  information,  guidance,  and  advanced  genetic  testing  to  help  you  cope  if  a  positive
         diagnosis of cancer occurs. If you are diagnosed with cancer, a dedicated Cancer Support Specialist will be assigned. The Cancer
         Support Specialist will conduct an orientation assessment and curate a personalized information and resource pack that is reviewed
         with you. Advanced DNA Testing & Second Opinion Pathology Reviews are made available at no cost to you, your partner, children,
         parents  and  parents-in-law.  All  results  are  provided  to  your  treating  physician.  A  treatment  course  will  then  be  set  by  your
         oncologist.
