Unum | Employee Assistance Program

The Employee Assistance Program (EAP) provides you and your household members with free, confidential assistance to help with
personal or professional problems that may interfere with work or family responsibilities and obligations. Services are available 24 hours a
day, 7 days a week via a toll-free nationwide number. You and your household members may be eligible to receive up to 3 face to face
sessions per issue, free of charge.

Go to www.unum.com/lifebalance or you may call (800) 854-1446 to be immediately connected to an EAP
counselor.

Travel Assistance

Unum | Worldwide Travel Assistance
Worldwide Travel Assistance, provided by Unum, offers a range of travel services to you and your eligible family members.
Available Services include hospital admission assistance, emergency medical evacuation, prescription replacement assistance,
transportation for a friend or family member to join a hospitalized patient, care and transport of unattended minor children,
assistance with the return of a vehicle, emergency message services, critical care monitoring, emergency trauma counseling,
referrals to Western-trained, English speaking medical providers, legal and interpreter referrals, passport replacement assistance,
and more. This program is available 24 hours a day, 365 days a year.
