SECTION FOUR: PAPERWORK AND DOCUMENTS



        TEAM MEMBER CLOCK IN/OUT FORM

        A Team Member fills out this form when he/she is unable to clock in or out at the Time Clock. As a

        Supervisor, it is your responsibility to go through these forms nightly and make the necessary
        corrections for your Team Member.

        If a Supervisor fails to make the necessary corrections, his/her Team Member’s time will not appear on
        the time clock report. This will result in additional work for the Supervisor and the bookkeeping
        department.


        This form needs to be turned into the HR Office when your shift ends. If there is no one in the HR
        office, please place these forms in the turn in box provided outside of the office.


                                                                                 You  must  make  sure  that  this
                                                                           form  is  filled  out  correctly  and  is
                                                                           legible for others to read.
                                                                                 Employees  First  and  Last  name
                                                                           must be filled out.

                                                                                 Fill  out  the  date  and  put  the
                                                                           Supervisor’s name where specified.
                                                                                 Fill out the TIME IN and TIME
                                                                           OUT  sections.  If  you  run  out  of slots,
                                                                           fill out another form.

                                                                                 Keep  this  form  with  you  and
                                                                           collect it any from your Team Member.
                                                                                 Turn    into   the   HR    Office
                                                                           immediately after your shift.
