DISIPLINARY-GUIDANCE FORM
This form is used for re-training purposes and warnings. The picture of this form is located on the next
page.
To fill out this form properly do as follows:
This form must always be legible if it is written out by hand. If it is not legible, you will be asked
to refill out the form. There is also a fill-able version of this form and it can be typed and
printed out.
At the top of this form, fill out the Team Member’s First and Last Name, date of the
Incident, and the area they are placed in.
Next, highlight whether this is a DISIPLINARY or GUIDANCE FORM. If you are filling out the
form on the computer, use the highlight option. If you are hand writing these forms use a
highlighter.
Next, select an option below that suit the incident being described. (Options such as Cell Phone
Violation, Insubordination, etc.)
Next, in the box, describe the misconduct or Incident as well as what is expected of the
employee in the future.
The question below this box is asking if the employee has been warned about this incident before.
Check YES or NO.
After that, there are four more checkboxes that this report constitutes. (Verbal Warning,
Probation, etc.)
The next writing box is where a Team Member is able to write his/her comments about the
incident. This is optional; the Team Member is not required to write a response.
The Team Member, HR Director and the Manager of Team Member’s area must print their First
and Last name, then sign and date this form.
Once this document has been filled out and signed by the Team Member and the Manager, this
form must be turned into the HR office 1 hour prior to the end of the Supervisor’s Shift. If
there is no one available in the HR Office, place that form in the turn in box provided outside of
the office.
Examples:
A Major Rule Infraction
Example – a no show/no call absence, being disobedient or respectful to Management, mistreating
customers.
A Major Safety Regulation Violation
Example – A ride operator leaving his ride while it is in operation or a Team Member jumping on
and off a ride while it is in operation.
After The First Verbal Warning Is Given
In this case we usually issue a written warning after a Team Member receives one verbal warning;
however, this may vary depending on the situation.
