TERMINATION FORM
The Termination Form (TQ Form) is filled out when a Team Member quits or is discharged. It is
imperative that this form is filled out completely and accurately.
To fill out this form properly do as follows:
This form must always be legible if it is written out by hand. If it is not legible, you will be asked
to refill out the form. There is also a fill-able version of this form and it can be typed and
printed out.
At the top of this form, fill out the Team Member’s First and Last Name, the effective date,
and the area they are placed in.
Next you will need to choose an option under “Type of Separation”.
Below that will also be more options under “Reason for Separation”.
Next there is a “Team Member Evaluation” section. This is used to evaluate the Team Member’s
performance while they were working with the company.
The next option is “Eligible for Rehire” depending on your evaluation and the circumstances, if
any, you will need to select if the employee is or is not eligible for rehire.
The “Additional Comments” section is for any other information you wish to add.
Once this document has been filled out and signed by the HR Director and the Department
Manager, this form must be turned into the HR office 1 hour prior to the end of the
Supervisor’s Shift. If there is no one available in the HR Office, place that form in the turn in
box provided outside of the office.
Terminating an Employee:
If an employee is terminated, paperwork must be filled out as soon as possible. This paperwork, the TQ
Form, must be turned into the HR Office as soon as possible; by law we only have 5 days to pay out any
terminated amployee.
