contact Human Resources. If you have any questions about this statement or your
rights under ERISA, you should contact the nearest area office of the U.S. Labor-
Management Service Administration, Department of Labor.
Benefits
Cliff’s offers group medical insurance programs for all regular full-time employees
working 30 or more hours per week the first of the month following 60 days of
employment. Cliff’s provides 100% of the eligible employee’s premium. Your
dependents coverage will be deducted from your paycheck (if applicable). Management
have dental and vision plan options.
Management reserves the right to change or delete plans as necessary.
All plan summary or other descriptions and contracts are available for review from
Human Resources.
Worker’s Compensation
On-the-job injuries may be covered by our Workers’ Compensation insurance policy.
This insurance is provided at no cost to you. If you are injured on the job, no matter
how slightly, report the incident immediately to your supervisor. Any employee who
receives treatment for a work-related injury will be subject to post-accident drug and
alchol screening. Any employee who receives treatment for a work-related injury, will
be subject to post-accident drug and alcohol screening. Workers' Compensation
benefits may be reduced if the use of drugs or alcohol contributed to a workplace
injury. Also, the company requires a formal written doctor’s certification for any work
related injuries before return to work. We ask for your assistance in alerting
management to any condition that could lead to or contribute to an employee accident.
Employees must notify management before seeking medical attention, unless it is an
emergency. Management will advise the employee of what medical facility to use, unless
it is an emergency situation. The employee will be given a form to take with him/her to
this facility (except in emergency situations).
A Workers' Compensation Report must be filed at the Main Office by the employee
within 10 days of the illness or injury.
