Life and AD&D



Group Term Life Insurance in the amount of $50,000, or two (2) times your base annual wage, rounded up
to the next $1,000, is provided at no cost to all full-time employees. In addition, the company provides
Accidental Death and Dismemberment coverage equal to $50,000, or two (2) times your base salary.

With your automatic enrollment in basic term life with Guardian, employees are eligible to earn “Tuition Rewards” through their

Voluntary Life


Voluntary Programs (100% Employee-Paid Benefits)

Voluntary term life insurance through Guardian is available to Employees and their dependents.
X Eligible Employees: 1 to 5 times your annual earnings up to $750,000. (Health statements are required

for employees that did not previously enroll during their new hire enrollment period or to increase their
original beneit.)
X Spouse coverage: increments of $10,000 to a maximum of $150,000. (Health statements are required for
spouses if the spouse was not previously enrolled during the employee’s new hire enrollment period or
to increase the spouse’s original beneit.)

X Child coverage: employees may elect child optional life in the amount of $2,500, $5,000, $7,500, or
$10,000. Children are eligible for this beneit from 14 days to 26 years. Children from 0 days to 14 days
are eligible for optional life in the amount of $500. The child beneit is per child unit; a unit can be one
(1) or more children.
