INTERVIEWING

This section contains questions to help you conduct a witness
interview. The questions are organized around the HFACS
framework.
Note that there are no direct questions pertaining to unsafe acts.
By asking the types of questions provided in this guide, you will
be able to better understand the underlying human factors that
preceded a given unsafe act. Later in your analysis, you will be
able to determine the types of unsafe acts that were committed.

WHO TO INTERVIEW?

● Participant witnesses - These are individuals who were
directly involved in the event and may include staff,
contractors or others at the scene.

● Eyewitnesses -These are individuals who were not
directly involved in the event but may have seen or
heard the event taking place. They may include
coworkers present at the time the event took place.

● Expert/Organizational witnesses -These are witnesses
who were not involved in the event but may have
pertinent knowledge about factors associated with the
event. Such individuals may be other workers
device/product manufacturers, supervisors or
administrators
