         the work processes related to the event? (Were these
         appropriate and current? Were they consistent with
         external guidelines or other internal policies? Were they
         clear and readily available to staff? How were they
         disseminated?)
      ●   Did management have an audit or quality control system
         to inform them of how key processes related to
         the event was functioning? (Was leadership engaged?
         Did leadership have an appreciation of operational risks?
         Was there a management plan for addressing risk and
         assigning responsibility for risk?) or is the relationship
         more adversarial/ strained?)
