Top 10 Tips to Maximize Your Plan Benefits
1. Choose a Primary Care Provider (PCP). Go to www.connecticare.com and click on Find a Doctor, or
contact Member Services at (860) 674-5757 or 1-800-251-7722, or via e-mail at info@connecticare.com.
Once you’re a member you can e-mail us through our secure messaging online.
2. Keep your ConnectiCare ID card in your wallet. Show it whenever you visit a participating practitioner
or fill a prescription.
3. Follow the referral rules of your plan. Find the referral statement at the bottom and center of your ID card.
Follow the instructions on the “Plan Coverage and Benefit Summaries” page.
4. Use participating practitioners and provider facilities in our network, unless you have an emergency
and you are 30 miles outside our Service Area.
5. File a claim if you use providers outside our network for emergency or urgent care. You may have to
pay for these services yourself and then submit a claim for reimbursement along with a note that explains
your urgent care/emergency situation. You have six months to file such claims.
6. Follow your Membership Agreement/Certificate of Coverage pre-authorization requirements. Certain
benefits and drugs need our “pre-authorization” or your benefits may be reduced.
7. Before seeking any behavioral health care, call 1-888-946-4658. For behavioral health treatments call the
phone number above or use the Find a Doctor tool on www.connecticare.com.
8. For quick facts, review your Benefit Summary. It’s a great overview of important details such as copays,
coinsurance and deductible amounts. Detailed plan benefits will be sent after enrollment.
9. Learn which doctors and hospitals are participating in our network. Practitioner participation in our
network changes from time to time, so be sure to check for the latest updates on our website.

quickest way to make sure you get the benefits you’re eligible for.
