Plan Coverage and Benefit Summaries
CONNECTICARE OFFERS A VARIETY OF HEALTH PLANS. SINCE EACH PLAN IS DIFFERENT,
IT IS IMPORTANT TO READ YOUR BENEFIT SUMMARY PROVIDED AFTER YOU ENROLL.
After you enroll, you will receive a new Member Kit
Your new Member Kit will include important information about your coverage. Be sure to explore
your Plan Benefit Summary and the How Your Plan Works flyer. That way, you won’t be lost when
it comes time to use your benefits.
Not sure where to find what type of plan you are in?
• The upper right-hand corner of your ID card tells you what type of plan you have.
• Your Benefits Summary will also explain what type of plan you have and how it works.
Emergency Coverage
In the event of an emergency, go to the closest emergency room immediately. A few examples of
emergencies include severe chest pain, trouble breathing, severe bleeding, poisoning or
seizures. Call 911 if you need help getting there. If at all possible, let your doctor know what’s
happening. Notify us at (860) 674-5870 as soon as you can, or someone on your behalf can contact
us if you are admitted into the hospital. (A friend or family member can call for you.)
ConnectiCare Dental Plans
ConnectiCare offers PPO-style dental plans with open access to broad participating provider
networks. Whether you are in a large or small group plan, Connecticare offers cost-effective dental
benefits, including out-of-network benefits and competitive rates. If you don’t have a ConnectiCare
Dental Plan, ask your employer to check out what we have to offer.
HMO Plans
Plans that require members to seek care from a participating practitioner and facility are called HMO
plans. Authorization is required before seeking care when services are being rendered outside of the
network. Members are encouraged to choose a Primary Care Provider (PCP).
POS Plans
Plans that allow members the choice of utilizing any health care provider are called POS plans.
Members who choose a ConnectiCare participating practitioner or facility will receive a higher level
of benefits. Members are encouraged to choose a PCP.
