EMPLOYEE AND DEPENDENT MEDICAL INSURANCE
All employees will be able to choose to have medical insurance for themselves and any eligible dependents. The
program allows you to choose from a listing of doctors and hospitals that will reduce your out-of-pocket costs or you
may go to any other doctor or hospital. Chartwell Hospitality pays 85% of the cost for this program for employees
and over 50% of the cost for covered dependents. The payroll deduction for this coverage will be:

EMPLOYEE AND DEPENDENT DENTAL INSURANCE
All employees will be able to choose to have dental insurance for themselves and any eligible dependents. The
program allows you to go to any dentist. Routine services such as exams are paid at 100%, restorative services such
as fillings are paid at 80% and major services such as crowns are paid at 50%. Chartwell Hospitality pays 85% of
the cost for this program for employees and 75% of the cost for covered dependents. The payroll deduction for this
coverage will be:
