VSP Standard, Premium, and Premium Plus Plans
The VSP Premium Plus Plan is an upgrade from the Premium plan! This plan will
give you the opportunity to use your plan allowances 2x in one year. For example, you
can use your $200 allowance for new frames or prescription sunglasses and your $150
allowance for contacts in the same plan year. Another example would be to use your
contact allowance 2x in the same plan year.

AEO provides you with a choice of affordable vision plans so you can choose the plan
that’s right for you. With VSP, Associates will enjoy more value and the lowest out-of-
pocket costs. You’ll also get the best care from a VSP provider, including a WellVision
Exam® — the most comprehensive exam designed to detect eye and health condi-
tions. When it comes to Choice of Providers, the decision is yours to make — choose
a VSP doctor, a participating retail chain, or any out-of-network provider. It’s easy to
find the perfect frame at a price that fits your budget.

Using your VSP benefit is easy. Create an account at vsp.com. Once your plan is
effective, review your benefit information. Find an eye care provider who’s right for
you. To find a VSP provider, visit vsp.com or call 1.800.877.7195. At your appoint-
ment, tell your provider that you have VSP. There’s no ID card necessary. If you’d like a
card as a reference, you can print one on vsp.com. There are also no claim forms to
complete when you see a VSP provider.

VSP covers visits to a VSP Laser Vision doctor before and after a Lasik procedure.
Associates are provided with a $375 per eye allowance for laser vision correction
through a VSP-contracted center or surgeon. Discounts average 15-20% off.
