Voluntary Supplemental Coverage
Team members are eligible to enroll in several coverages through Prudential. These coverages include
accident, critical illness and hospital indemnity. Prudential will pay a cash benefit directly to you. The amount
you will receive depends on the type of injury and care received. You can use the money from Prudential
any way you like—such as everyday expenses like groceries or any out-of-pocket medical costs.
These coverages are not designed to replace your medical coverages, but offset any out-of-pocket expenses
you have.
Critical Illness Insurance

Team Members can keep their coverage at the same rates and benefits if
