Disability Insurance Plan Highlights


        Disability insurance provides income replacement in the event you are unable to work due to a
        non-work related illness or injury. Plans are separate from any state disability plans you may be eligible for.

        If you enroll or make changes to your disability coverage during the Annual Enrollment period
        or outside your initial enrollment (if a new hire or rehire), you may be required to provide evidence
        of insurability (EOI).
