Life and Disability Insurance
Protect Yourself and Your Family
Benefit eligible Team Members are automatically enrolled in Basic Life, and Accidental Death and
Dismemberment (AD&D) insurance at 1 times your salary (minimum of $30,000) paid 100% by SSC.
You can elect to purchase supplemental and dependent life insurance options. The life insurance options you
choose help protect your family from financial hardship. If you smoke, you will pay more
for supplemental insurance.
If you enroll or make changes to your life insurance coverage during the Annual Enrollment period
or outside your initial enrollment (as a new hire or rehire), you may be required to provide Evidence
Of Insurability (EOI).

Designate A Beneficiary
Benefits from your Basic Life and Supplemental Life insurance coverages are paid in a lump sum
to the beneficiary you designate. You can update your beneficiary at any time by logging onto
benefits.vcf.com. You are automatically the beneficiary on your Dependent Life coverage.
