Life and AD&D Insurance – The Hartford
Life insurance benefits can provide your family with peace of mind. Murata provides you with basic life insurance benefits
and basic accidental death and dismemberment (AD&D) insurance, each equal to your annual pay, up to a
$500,000 maximum. AD&D coverage provides a benefit in the event of accidental death or a serious injury. Murata pays
the full cost of basic life insurance and basic AD&Dcoverage.
If you need additional protection, you may purchase voluntary life insurance and AD&D coverage for yourself. You may
purchase voluntary life insurance up to $500,000 with a Guaranteed Issue of $100,000. If the total of your voluntary life
election is over $100,000, you must complete an Evidence of Insurability (EOI) form for submission to The Hartford
underwriters to be approved.
Murata offers spouse life in increments of $10,000 (up to $50,000), and child life in the amount of $5,000, per child. If
your dependent child is 19 years of age up to 25 years of age, they must be single and a full-time student. You pay
the full cost of all voluntary insurance coverage. You must elect voluntary life to enroll in spouse or child life.
Disability Insurance – The Hartford
If youshouldrequireanextendedabsencefromworkduetoanillness or injury,Murata provides youwithaccessto disability
benefits, so that you can focus on your recovery. Peace of mind during an illness or injury can assist in the healing process.
Murata’s short-term disability (STD) plan can offer you up to 100% of your pay for up to 12 Weeks. Murata pays the full
cost of your STD coverage. STD benefits are based on your years of service with the company. If you have:
• Fewer than five years of service, you receive 60% of your pay for up to 12 Weeks.
• Five years to less than 10 years of service, you receive 100% of your pay for the first 6 weeks and 60% of
your pay for the remaining 6 weeks.
• 10 years or more of service, you receive 100% of your pay for 12Weeks.
If your illness extends beyond 12 Weeks, long-term disability (LTD) coverage will pay 60% of your pay, up to $10,000 a
month. Murata pays the full cost of LTD coverage.
Retirement Savings Plan – Fidelity Investments
You have the opportunity to save for your retirement through our 401(K) savings plan. You can contribute up to $20,500
IRS maximum and $6,500 catch-up for employees age 50 and older.
If you are age 50 and older, you may contribute from 51% up to 100% due to catch-up.
The company matches 100% up to 6%. Your contributions and the company match are both immediately vested at 100%.
