Hospital Indemnity – Allstate

         Health insurance is a good way to help pay the costs of medical treatments that may be necessary, such as treatment that requires a
         visit to your physician, the emergency room, or an extended stay in the hospital. Even just a couple of days in the hospital could be
         costly to you. Lost income, medicines and therapies can quickly add up.

         Supplemental health insurance like Hospital Indemnity with Allstate can help protect your hard-earned savings, and it can help you
         cover some of the costs associated with hospital care.

         Using the “best in class” approach to identify the most relevant and competitive voluntary benefits, Murata offers a corporate group
         contract for Hospital Indemnity to our employees. The Hospital Indemnity insurance offered by Allstate provides cash benefits to
         offset out-of-pocket expenses associated with medical services that would otherwise cause additional financial burdens Allstate will
         require an Evidence of Insurability (EOI) form to be completed, if you are not a new hire enrolling for the first time for thisplan.

         This coverage also has a payment scale within individual policies that are mailed to your homes. Rates are locked and smoker rates
         do apply. This policy is portable, which means you can take this policy with you if you leave the company, up through COBRA period.

         Critical Illness Insurance – Allstate
         Allstate Benefits (AB) group voluntary critical illness coverage provides a lump-sum cash benefit to help you cover the out-of-pocket
         expenses associated with a critical illness.

         No one knows what lies ahead on the road through life. Will you have to undergo a major organ transplant or coronary artery bypass
         procedure? Will you suffer a stroke or a heart attack? The signs pointing to a critical illness are not always clear and may not be
         preventable, but this coverage provides financial protection in the event you are diagnosed.

         Critical illness coverage offers peace of mind when a critical illness diagnosis occurs, and payments are made directly to you. The
         coverage is a fully portable plan that offers payment when a major condition is diagnosed. Employees are responsible for the total
         cost of this pre-tax benefit.

         Policies are mailed to your home. Rates are age-locked and this policy is portable, which means it’s able to continue if you leave the
         company. Please be sure to read and understand the content of your policy.

         Long Term Care (LTC) Plan

         Long Term Care (LTC) insurance will assist with paying for custodial care received in an assisted living facility, nursing home, or in
         your home if you need assistance with toileting, continence, bathing, dressing, transferring, eating, or supervision due to cognitive
         impairment.
