A Guide to Your Benefits | 2022



       Goodwill provides the following benefits based on employee eligibility. Upon meeting eligibility, you are

       automatically enrolled at no cost to you.

       Basic Life and AD&D Coverage – United Healthcare

        Basic Life and AD&D Insurance: $50,000                   Basic Life and AD&D Insurance: $20,000
        All management salaried employees working 30+            All other regular full-time employees working 30+
        hours per week                                           hours per week

        Dependent Life Insurance
        All regular full-time employees working 30+ hours per
        week

        Spouse  $5,000


        Child      $2,000 (age 15 days to 26 years)



       Short Term Disability (STD) – United Healthcare

        All regular full-time employees working 30+ hours per week will receive coverage with the ability to
        ‘buy-up’ by paying an additional premium to gain greater protection and a larger weekly benefit.

              Basic coverage of 50% of weekly earnings to a maximum of $200

              Buy-up Option 1- provides 50% weekly earnings to maximum of $500
              Buy-up Option 2- provides 60% weekly earnings to maximum of $1,900
              Elimination period for injury or sickness is 14 calendar days
              Benefit duration of 11 weeks

       Long Term Disability (LTD) – United Healthcare


        All full-time employees working 30+ hours per week with one year of service.


              Basic coverage of 60% of monthly earnings to maximum of $10,000
              Minimum benefit is greater of $100 or 10%
              Elimination period is 90 calendar days
