       LIFE & FINANCIAL BENEFITS | there when you need it!



       OPTIONAL BENEFITS
       If you need additional protection for your family, the following optional coverage options are available to all full-time employees working 30+
       hours per week.

       Optional Supplemental Life and AD&D Insurance for Employees – United Healthcare
            Employees can apply for amounts in $10,000 units as with an overall maximum of the lesser of 5 times annual earnings or $500,000.
            Guarantee Issue Amount (new hires) of $130,000. Additional coverage subject to Evidence of Insurability (EOI).
            Supplemental life insurance benefit will reduce 35% at age 70, and an additional 15% of the original amount at age 75.

       Optional Supplemental Life Insurance for Spouse and Child(ren) – United Healthcare
            Spouse – Amounts in $5,000 benefit units as applied for by the employee. Overall maximum – the lesser of 100% of the employee
            Life/AD&D amount or $100,000.
            Guarantee Issue Amount (new hires) of $25,000. Additional coverage subject to Evidence of Insurability (EOI).
            Age reductions are the same as the employee as outlined above.
            Child (each) – Amounts in $2,000 benefit units as applied for by employee. Overall maximum $10,000 with a guarantee issue of $10,000
            for new hires. Age 15 days to age 26 years: up to $10,000.

       Optional Critical Illness Protection Plan – United Healthcare
       Pays a lump sum benefit if an insured is diagnosed with the first occurrence of a coverage-specific critical illness
            Plan includes $10,000 for employee and $5,000 for spouse and child and is offered with tobacco and non-tobacco rates.
            Basic plan covered conditions include heart attack, stroke, major organ transplant, permanent paralysis, and end-stage renal failure.
            Also includes 18 conditions with neurological benefit s such as Advanced Alzheimer’s, Advanced Parkinson’s, ALS, Advanced Multiple
            Sclerosis.
            You can take this coverage with you should you leave Goodwill.

       Optional Accident Protection Plan – United Healthcare
       Helps employees meet the out-of-pocket expenses and extra bills that can follow an accident injury, whether minor or catastrophic
            Benefit payments may be made for emergency room care, doctor office visits, and physical therapy, to name a few.
            The accident basic plan is guarantee issue; therefore, no health questions are required.
            AD&D Benefits of up to $50,000
            And, just like the Critical Illness Protection Plan, you can take this coverage with you should you leave Goodwill.

       Optional Travel Assistance Insurance for Employees – United Healthcare
            Provides access to emergency medical help, travel services, and pre-departure information.
            Available when you’re more than 100 miles away from home.
            All services must be arranged with Generali Global Assistance, Inc. in advance.


       Employee Assistance Program – United Healthcare Employee Assistance Program
       Available to ALL Goodwill Columbus employees working 30+ hours. When you have questions, concerns or emotional issues
       surrounding your personal or work life, you can count on us to offer help.
            United Healthcare’s Employee Assistance Program (EAP) offers unlimited access to master’s-level consultants by telephone, resources
            and tools online, and up to three face-to-face visits with a consultant for help with a short-term problem. You can also:
              o   Speak with financial experts by phone regarding budgeting, controlling debt, investing for college and preparing for retirement.
              o   Work through complex, sensitive issues such as personal or work relationships, depression, grief or issues surrounding substance
                  abuse.
              o   Get a referral to a local attorney for a discounted fee or telephonic legal consultation; if you decide to retain the attorney, you
                  may be eligible to receive a discount on additional services.
