Eligibility Full-time employees of Skyworks working 30 hours or more
per week are eligible for most benefits effective upon date of
hire.
Regular employees working less than 30 hours are eligible for
the 401(k) plan, Employee Stock Purchase Plan, the Employee
Assistance Program and Health Advocate, prorated vacation,
holiday and wellness time.
Interns and temporary employees working 30 hours or more
are eligible for medical benefits, the Employee Assistance
Program, Health Advocate, holiday and wellness time.
Dependents
Eligible dependents include your spouse and child (ren) up to
age 26 unless noted otherwise. Dependents older than age
26 and considered disabled while meeting certain criteria can
continue to be eligible for coverage. To request coverage for
a disabled dependent, please fill out the Request for Retaining
Coverage for a Disabled Dependent Child form. Dependent
verification documents are required to add a dependent to
your benefit plans.
Domestic Partnership
Skyworks recognizes that families come in all shapes and
sizes and offers domestic partner coverage for medical,
dental, vision, life and AD&D. A domestic partner must be
financially interdependent, living in the same quarters and
in a committed relationship at least 6 months. To qualify, an
Affidavit of Domestic Partnership is required. In the event
you wish to remove your domestic partner from your benefits
plans, please complete the Domestic Partner Termination
form.
