How Do I Enroll Once you’ve started, you will receive an email confirming
your ability to enroll online. You should enroll as soon as

possible but no later than 30 days beyond your hire date. The
sooner you enroll, the sooner we can activate your coverage.
Beneficiaries for life and AD&D can be assigned online. You
can designate a beneficiary for your 401(k) account on the
Fidelity website.

Identification You can expect to receive your medical and/or dental ID cards
in the mail 10-14 days following receipt of your enrollment
Cards form by HR Global Services.

What should you do until you receive your new ID card in the
mail? If you need medical attention prior to receiving your
ID card, you can use a copy of your benefit confirmation
statement as your proof of coverage when setting up an
appointment with your doctor. Your pharmacy may accept
the confirmation page as proof of coverage or may request
that you pay for your prescription at the time of purchase.
If the latter applies, save the receipt and submit it to the
address on the back of your ID card when it arrives. Your plan
will reimburse you for expenses incurred, minus applicable
co-payments. Plan Numbers can be found on Benefits
Connection.

Delta Dental issues family cards with only the employee name
on the card.

Vision Service Plan (VSP) does not issue ID Cards as
participating providers will have enrolled employees in their
systems.
