Breaches in safety protocol and procedures - Actual Causation
• There was no pre-scenario safety check or active scenario monitoring. This passive approach contributed to a failure to
control the overall safety of the force-on-force environment, which ultimately resulted in the victims injury.
• The event started without warning, i.e. there was no countdown or verbal “going hot”, etc. Consequently the victim was in
the process of adjusting his mask which had become dislodged when he was shot in the eye.
• There were no instructors or safety personnel inside or monitoring the room, or positioned with the ability to call a cease
fire and stop the scenario before the victim was shot.
• On the day of the incident the victim was never briefed on emergency mishap procedures, including what to do if his safety
equipment became dislodged.
• Had the victim been instructed on emergency mishap procedures, there is a high probability that he would have been
able to prevent his injuries.
