ADMISSION REQUIREMENTS FOR NEW AND
                                         RETURNING PUPILS/STUDENTS

            The  following  documents  are  requirement  for  admission  in  the  basic  education
            department;

               1.  Original Form-138 or Report Card with Learner’s Reference Number
               2.  Recently issued Certificate of Good Moral Character with school seal duly signed
                   by the school principal or guidance counselor.
               3.  Recently Issued Birth Certificate from Philippine Statistics Office. If not available,
                   a recently issued Birth Certificate from the local Civil Registry is acceptable (for
                   Basic Education only)
               4.  Age requirement per grade level follows what is prescribed by the Department of
                   Education.
               5.  If applicable, Certificate of Honors indicating the honors received and the number
                   of graduating students signed by the School Principal with school seal.
               6.  For grade 6 graduates on ESC application, you may email the Admission office on
                   the requirements to qualify for tuition fee subsidy in Junior High School.
               7.  For  ESC,  Public  Voucher  and  QVR  application,  you  may  email  the  Admission
                   Office on the requirements to qualify for tuition fee subsidy in Senior High School.
                   For  parents  of  APCAS  student  graduates  of  Grade  10,  the  Junior  High  School
                   coordinator will assist in providing guidance on how to apply online for tuition fee
                   subsidy.

            All documents stated above are initially to be sent in soft copy to our Online Admission
            Application Portal. All original copies are to be submitted via any courier service/mail or
            can  be  submitted  in  person  to  the  Admission  office  in  our  school  immediately  after
            completing  the  online  enrollment.  Policies  and  procedures  on  Social  Distancing  and
            Health Safety shall be strictly observed while in school.

            Also, all documents  submitted as part of the requirement for admission to the school’s
            basic education department such as but not limited to Form 138 ( Report Card/Form 137/
            Permanent Record of Grades)/Birth Certificate shall not be returned to students for any
            reason  including  withdrawal  of  enrollment  or  transferring  to  another  school  and  for
            personal use.
