                  CCS may monitor content out on the Internet. Policy violations may result in discipline up to and including
                    termination of employment.
             Training: All new contractors are trained on Cybersecurity and the IT policy within the first 30 days of hire. All
             contractors will receive a refresher’s training on Cybersecurity and the IT policy annually.
             20. Records Management

           CCS will maintain clinical records related to the provision of EDWP (CCSP and SOURCE) services in accordance with
           accepted professional standards and practice and with the standards in the EDWP (CCSP and SOURCE) General
           Services Manual. Records will be made available to DCH and their agents as requested. All records will be
           maintained within the state of Georgia. Records are maintained in a manner that is:
               •  secure
               •  accurate
               •  confidential
               •  accessible

            20.1 Emergency Information:
            CCS will maintain written emergency information on each member. The emergency information must be easily
                accessible in the member’s record and, at a minimum, include the following:
               •  name and telephone number of the member's attending physician
               •  member's hospital preference
               •  names and phone numbers of member's representative and other emergency contacts
               •  Known medication/pertinent medical information, including allergies

           CCS will ensure Supervision of Services by a Registered Nurse (606.17), That the Member Care Plan (606.18)
           includes all Necessary items as outlined in (606.18) and Progress/Clinical Notes (606.19) sections of the EDWP
           (CCSP and SOURCE) General Services current manual.
           CCS will maintain



            20.2 The member referral packet includes:
               1.  Copy of Level of care and Placement Instrument, signed and dated by the physician
               2.  MDS-HC V9 (includes Caps/ILOC and DONR for CCSP)
               3.  Client Detail/Demographics (CCSP) or SOURCE Referral Form (Rev 10/2019)
               4.  Appendix I- SOURCE (Rev 10/2019)
               5.  Care Plan assessment including Service Order and Task Lists/initial and as updated –(CCSP) (Rev 1/2015,
                  4/2017, 10/2019)
               6.  Carepath/Member Version (SOURCE) (Rev 10/2019)
               7.  Copy of signed Authorization for Release of Information and Informed Consent (Signature Page)
               8.  If client is MAO or PMAO, copy of the completed Potential EDWP (CCSP and SOURCE) MAO Financial
                  Worksheet, which contains client signed Statement of Intent: Cost Responsibility.
               9.  Any other relevant information, including:
                      a.  Psychological and Psychiatric evaluations
                      b.  Information about client that the provider needs before completing an evaluation/assessment
                      c.  Copy of the DCH Authorization for Release of Information, if applicable (See Appendix L of the EDWP
                         (CCSP and SOURCE) General Manual)
