                                              Policies and Procedures
              Medical Assistance Only (MAO) Members: 0TA Medical Assistance Only (MAO) member is one who receives
              Medicaid benefits but who receives no cash assistance such as Supplemental Security Income (SSI). MAO
              members may be required to pay toward the cost of EDWP (CCSP and SOURCE) services (cost share).


         12.3 Onboarding
             Crew Consulting Services LLC will follow a standard onboarding procedure when admitting all individuals to
             the agency. The procedures are followed by all care coordinators to ensure all information is captured for the
             Person-Centered process. Before admission to The CCS Organization, the individual must meet criteria for
             admission, which is outlined specifically by each state.

           Procedures

                Accepting New Cases

             •  Crew Consulting Services LLC will offer services to individuals who meet the criteria for admission and for
                whom the agency can provide appropriate services.

             •  Crew Consulting Services LLC will then determine if the agency can meet the individual’s needs. If so,
                the admission process will continue.

             •  Once the agency has made the decision to accept the individual, the Care Coordinator and or a CCS
                representative will make the initial contact with the individual/family member and or legal guardian within
                no more than 2 business days.
             •  The Care Coordinator will review the individual’s file and will schedule a meeting with the individual/family
                member and or legal guardian and the support as needed.

                Onboarding

             •  Once a decision is made to serve the individual, the onboarding process will begin.

             •  Once the file is assigned to the Care Coordinator, he/she will make initial contact within 1 business day. If the
                individual cannot be reached, the Care Coordinator must continue their efforts to do so every 48 hours. If the
                Care Coordinator is unable to reach the individual/family member and or legal guardian within 1 week, the
                office the State office to verify contact information and request assistance. After 2 weeks, a certified letter
                will be sent outlining our efforts to contact. All efforts to reach the individual/family and or legal guardian
                should be documented in support notes.

             •  The Care Coordinator and/or a CCS representative will schedule a meeting with the
                individual/family member and or legal guardian.

             •  During the initial meeting, the Care Coordinator or other CCS representative will review care coordination
                services with the individual. The individual shall have the opportunity to ask questions about what care
                coordination is, what s/he can expect from the Care Coordinator, and what will be expected of the
                Individual.

             •  The Individual should be provided with contact information for the Care Coordinator as well as the Care
                Coordinator’s supervisor.

             •  The following should be reviewed with the individual and/or legal guardian and signed, as applicable:
