• Document information surrounding the “What, When Where, Who, and How”
regarding the incident.
• Detailed descriptions of any injuries.
• An objective reporting of what the Individual stated or indicated.
• What you did, who you notified.
• Any reporting of second-hand information should be reported as such. Do not report it as fact. (i.e.
“Staff reported to me that….”) Always proofread your documents for correct grammar and spelling.
We will not only adhere to the CCS Critical Incident reporting policy (CC-005-03),
but we will also adhere to DBHDD’s incident reporting policy (Policy 02-440).
11.4 Individual Rights
Crew Consulting Services LLC values each person’s rights as a citizen. Each person we serve will be given the
opportunity to exercise their rights. Should any right be violated, a process of grievance and appeal will be
utilized.
CCS recognizes that all people:
Are unique, have value, and must be afforded dignity and respect.
Can contribute to family and society, provided the necessary services, and supports.
Can lead self-determined lives when given freedom, responsibility, support and control over their lives.
Have the right to participate fully in developing the unique services and supports that allow them to
pursue their dreams, hopes, and desires.
Can benefit from the ongoing support of family and friends, i.e., a circle of support.
Have a right to the same life choices as others.
Have a right to be safe and free from harm.
Can, with support, participate fully in society and should not be expected to earn that right by progressing
through a “continuum of care”.
The right to be informed of and participate in preparing the Person-Centered Individualized Service Plan
(ISP) and any changes in the Plan of Care.
Must have ready access to vital services and supports, including accessible transportation, affordable
housing and health care, competitive work, inclusive recreation and leisure opportunities, and
specialized services, as well as those services generally available to all who reside in a community.
Have the right to belong to and be included in all aspects of society while retaining their own
cultural and ethnic heritage.
Benefit from inclusive communities, just as society benefits from the contributions and economic
productivity of those with disabilities.
