                                  Safety plays a very significant role in our life – whether at work, on the road or
                                  at  home.  It  has  been  found  that  a  majority  of  accidents  are  because  of
                                  ignorance, negligence, apathy, bad habits and wrong attitudes. Thus, accidents
                                  can be prevented if people are made aware of risks, safety precautions and the

                                  benefits accruing from doing things in a safe way. Identification of Nearmiss
                                  Incidents,  i.e.  incidents  which  have  potential  of  converting  into  accidents  is
                                  essential to avoid accidents.


             Analysis of Nearmiss incident helps us to locate deficiencies in the system. It certainly provides an
             excellent learning opportunity at no cost as well as an opportunity to rectify the inherent shortcomings.
             A majority of accidents occur because similar incidents which happened earlier had not been heeded
             to. Studies have revealed that analysis of Nearmisses and corrective actions done to rectify them has
             helped in reducing the number of life threatening situation in industry to a very large extent.



             This compendium encompasses Hi-Potential Nearmiss incidents which were identified in our plants
             and the lessons learnt from these incidents. I am sure that all plant personnel would  benefit from
             the sharing of these incidents, helping in spreading of safety awareness  amongst all.


             I appreciate the effort taken by F&S team in carefully compiling the major Near miss incidents.
             booklet should be considered as an inspiration to report all the misses/unsafe conditions In the
             future so that timely action can be taken to rectify the unsafe situation/unsafe action.
