Case 3 Date: 14.09.2018 Time: 09:45 hrs.

Incident

Excavation job was being carried out for foundation work through JCB/Poclain in caustic area.
Fire/smoldering observed in left out wax in the drain of caustic area. It was also observed layer of wax
found on the outer shell of pipe line near fire site.

Root Cause

Left out wax which was not removed due to approach constraint and below pipes in trench might have
generated smoke due to spark generation by poclain during concrete breaking.

Recommendations

1. Trench to be filled with sand before breaking as complete wax removal could not ensured
due to approach constraints. Just blocking of trench is not sufficient.

2. Water spray is required to keep the area wet at the point of contact of poclain with concrete.

Case 4 Date: 14.11.2018 Time: 17:32 hrs.

Incident

150 T Crane was parked for tube bundle removal of E-1501 and boom was extended to desired height.
Crane operator left the crane with extended boom without locking the centre post gear. Suddenly boom
started swing in horizontal plan. Immediately operator was called & boom movement stopped.

Root Cause

Casual approach by crane operator; he forgot to lock the centre post gear and left the site.

Recommendations

1. Crane operator to be counseled for behavioral improvement and casual approach on duty.

2. Agency to ensure that no matter of time crane should not be unmanned without locking of gear. Time
to time skill development for crane operator to be ensured by competent agency.

3. Rs. 10000/- to be imposed to M/s-Prakash Const. for negligence of operator and on account of safety
concern.
