Case 2 Date: 14.05.2018 Time: 11:00 hrs.
Incident
One tower mounted remote operated HVLR no.3
testing was in progress, when water was charged,
suddenly HVLR motor got detached from it's
original position & fell down on the floor from the
tower due to jerk. Immediately operation was stopped
& area was barricaded.
Root Cause
1. Excessive vibration while operating the ROLRM.
2. Restriction in movement of Jet/Fog assembly of ROLRM-03 – As seen from the site snap of the jet / Fog
nozzle,debris particles are found on the nozzle outlet, which is creating restriction in movement of
Jet/Fog nozzle led to excessive load/pressure on motor as well as base frame of Jet/Fog Nozzle.
Recommendations
1. 1. It is proposed to provide MOV operated HVs (in place of SOV) with bypass line for operating the all
ROLRMs of PP Unit (4 Nos.) as its operation is very smooth because of its controlled opening. It has
been verified with MOV operated ROLRMs of Swing Unit.
2. The debris materials observed at the nozzle to be cleaned periodically. The same to be included in the
checklist of PM jobs.
3. Filter to be provided in the SOV/Pneumatic relay vent port to ensure proper operation of HVs.
4. ROLRM operation checking is to be done from its local start panel, not from SOV knob of HVs.
