Case 1 Date: 16.04.2018 Time: 13:17 hrs.
Incident
A full load Propylene Tanker got overturn just outside PNC gate no-1, near CISF barrack and started minor
leaking from Roto-gauge. Affected area barricaded & water curtains laid. Over Turned tanker was safely placed
its upright position with help of Cranes and Hydra from PNC. By M/M & then Propylene leak temporarily
arrested by LPG Marketing Team & finally shifted/escorted safely to PTD Gantry along with Fire Tender
for unloading at 1800 hrs.
Root Cause
1. Inner circle (Sharp turn) maneuvered at high speed. Marks available at site depicts that tanker turns
within the inside portion of road.
2. Driver was wearing slipper while driving. Using slippers while driving not recommended as it would be a
potential cause of accidents if slippers got stuck with any of the pedals (accelerate/break/clutch).
3. No speed restriction along the peripheral road.
4. Poor driving Knowledge and skill of Tanker driver.
Recommendations
1. Road width can be increased at turn, but despite increasing the width/ turning radius, if driver uses the
inner circle as in prevalent case that too at high speed, it may still cause overturning of vehicle.
2. Speed breakers in upstream side of road approx 30m prior to culvert can be provided. It may cause
inconvenience to regular users as well for smooth running of vehicles during emergency but can avoid the
cases of overturn at turning portion cause of high speed. Even if the vehicle uses inner circle/sharp turn,
cause of breaker its speed remain slow, which may avoid overturn.
3. Board depicting Sharp Turn ahead/Drive slow as well speed breaker ahead to be provided on both sides
4. Safety training of Drivers & their helpers to be carried out at regular frequencies (monthly).
5. Penalty for wrong doers/agency OR some token appreciation for safe path followers can be initiated.
6. Meeting with tankers owners/transporter officials to be conducted and counseled to prevent
reoccurrence of incident in future.
