Where will I see my Explanation of Benefits (EOB)?
You will be able to view this through the Centivo app or member portal, available at my.centivo.com. You’ll also
have the option to receive a paper EOB from Centivo in the mail (paper EOBs will be the default).

FAQs specific to the MBA Centivo Plus Plan

What is the network for the MBA Centivo Plus Plan?
The Centivo Network is built around local high-quality providers in Connecticut and Michigan.

In Connecticut, the network is comprised of high-quality healthcare providers across metro NYC, CT, and NJ
from leading health systems and independent practices such as Nuvance Health and Trinity Health of New
England, for access to local primary care physicians, specialists, urgent care centers, labs, and imaging centers.

In Michigan, the network is comprised of high-quality healthcare providers from Ascension and Trinity Health, for
access to local primary care physicians, specialists, urgent care centers, labs, and imaging centers.

Please note: Centivo’s Michigan network is new for 2021, launching specifically for MB Aerospace employees. As
such, your doctor may not be immediately familiar with the Centivo Network. Rest assured that if your doctor is
listed in the Centivo Network, they are part of the network. For any network questions, you or your provider can
always call the Centivo Concierge Team at 844-599-0604.

Where can I look to see if a provider is in the Centivo Network?
Before the plan year begins, you can view the Centivo Network provider directory at https://mbae.centivo.com.
Once the plan year begins, you will be able to view the Centivo Network through the Centivo app or member
portal, available at my.centivo.com. You can also call the Centivo Concierge Team at 844-599-0604 if you need
help finding a provider.
Is there out-of-network coverage available?

The MBA Centivo Plus Plan is an in-network only plan, and as such, does not provide coverage if you go to an
out-of-area provider, except for emergencies and urgent care visits.

What if I have a dependent in college out of the area? Can they use the MBA Centivo Plus Plan?
The MBA Centivo Plus Plan does not provide any coverage if you go outside the network (except for emergency
care or urgent care visits when traveling out of the area). If you have any out-of-area dependents, such as a child
in college, you should consider whether this plan is adequate for your family.

Why is the MBA Centivo Plus Plan only available to employees in Connecticut and Michigan?
The MBA Centivo Plus Plan uses a local network of providers in order to deliver higher-quality care to MB
Aerospace employees and their families. This network, the Centivo Network, is currently available in Connecticut
and Michigan. For employees who do not sit in Connecticut or Michigan, the MBA HSA Plans offer a national
provider network to ensure you have access to providers near where you live.
