FAQs specific to MBA HSA High and MBA HSA Low Plans, utilizing the Cigna network

Are the MBA HSA High and MBA HSA Low Plans the same as the HDHP / HSA Plans offered last year?

The MBA HSA High and Low Plans have the same benefits as the HDHP / HSA High and Low Plans offered last
year. However, the main differences this year are:

1. The benefits are being administered by Centivo. As such, you will receive a new Centivo ID card in the mail,
your Explanation of Benefits (EOBs) will come from Centivo, you will view your benefit details on the Centivo
app or member portal, and you will call the Centivo Concierge Team for any benefit questions.
2. The provider network is different. We will be utilizing the Cigna PPO provider network, which offers access to
over 1 million healthcare providers across the US.

Where can I look to see if a provider is in the network?
Before the plan year begins, you can utilize the Cigna provider search tool on their website,
https://hcpdirectory.cigna.com/. Once on this site, you can begin your search by entering your address, city,
or zip code, and searching by doctor type, doctor name, or location. You will then be prompted to enter your
search terms, and then will be given the option to log in or search as a guest - choose “guest”. Lastly, you will
be asked to select a plan - choose “PPO”.

Note: Once the plan year begins, you will be able to create an account at mycigna.com using your Centivo
member ID number. You can go directly to mycigna.com or if you’re logged into the Centivo app or member
portal, there will be a link to take you to mycigna.com.

How can I find a Cigna Care Designated Provider?
When browsing the Cigna provider directory, simply look for the Cigna Care Designation symbol which will
appear in the provider search results for the applicable providers:

Can I estimate costs of procedures in mycigna.com?
Yes, over 200 procedure codes are available for you to estimate your costs. You must be logged into
mycigna.com to see these details. Please note that all benefits and eligibility questions should be directed to
Centivo. Members can reach the Centivo Concierge Team at 844-599-0604.

Is there out-of-network coverage available?
Yes, both MBA HSA Plans offer out-of-network coverage. However, please remember that out-of-network care
will cost you more out-of-pocket, with a higher deductible, higher coinsurance, and higher out-of-pocket
maximum.

Will MB Aerospace offer an HSA contribution if I enroll in the MBA HSA High or Low Plan?
Yes, as in previous years, MB Aerospace will contribute to your health savings account (HSA) if you enroll in
the MBA HSA High or MBA HSA Low Plan. The contributions are $500 for employee-only coverage, $750 for
employee + 1 coverage, or $1,000 for family coverage.
