What does it mean to activate?
You will need to activate at the start of the plan year to receive the full level of coverage under the MBA Centivo
Plus Plan. Activation is the process of creating an account with Centivo and designating a primary care doctor
for you (and any family members covered under your plan). During activation, you will also have the opportunity
to learn more about your plan. You can activate by downloading the Centivo app, through the Centivo portal, or
by calling the Centivo Concierge Team to walk you through activation. You will receive information in the mail on
how to activate in late December.
Who needs to activate?
All participants must activate in order to receive coverage (except for OB/GYN, behavioral health, and
emergencies). If one family member is activated but others are not, only that one family member who is
activated will receive coverage for care (emergencies are always covered whether you activate or not).
How do my dependents activate?
Adult dependents (age 18 and up) may activate on their own through the app, portal, or by calling the Centivo
Concierge Team. Alternatively, you may activate for them through your account. (Please note: The primary
account holder must activate/select a PCP for any dependents under the age of 18.)
When is a referral required?
You must get a referral from your Primary Care Team before visiting any specialists, with the exception of OB/
GYN and behavioral health. If you do not get a referral, your specialist care will not be covered under this plan.
While your Primary Care Team is urged to provide referrals to in-network specialists only, it is ultimately your
responsibility to ensure your visits are to in-network providers.
If the doctor to whom I’m referred to is not available, can I go to another doctor of the same specialty?
Referrals allow you to get care from a particular specialist for a set number of visits or period of time. If you want
to see a different doctor, you must get a new referral.
What if I already have an appointment scheduled with a specialist in January?
If you already have a specialist visit scheduled for early in the plan year, be sure to contact your Primary Care
Team and get a referral from your team to ensure your visit will be covered.
What happens if I go to the doctor before I activate?
You must activate before going to the doctor. If you don’t activate, your care will not be covered (except for
OB/GYN, behavioral health, and emergencies).
Can I change my PCP during the year?
You can change your designated PCP any time in the Centivo app or portal, or by calling the Centivo Concierge
Team. Please be sure to change your PCP designation before going to your new Primary Care Team or your PCP
visit will not be covered.
