If we look at the loss-makers then an average practice would lose £79.56, however
when we factor in all the concessions then the overall profit still averages £393.55.
In other words, you win some you lose some, but the overall balance is in your favour.
Good job because really you can do little about the over concession lines anyway.
This brings me nicely to the other frequent question raised – when should we
endorse and more importantly why? Often endorsing is being done to try and
counter concession prices. No point – let me explain why.
Endorsing Prescriptions in 2024.
Nowadays endorsing prescriptions is practically a thing of the past. Great
news, so please, those of you who have been endorsing all your prescriptions
for the past 20 years, and still do so, please read on.
So how do we know what to endorse. Well really, 99% of prescriptions
require no endorsement and those that do tend to be for more unusual items
such as specials. But let me give you the facts in as simple a format as I can.
The table below should help.
General endorsements
Prescribed item Required endorsement
A generic medicine listed in Part VIIIA of the Drug Tariff. • Pack size — where multiple pack sizes are listed.
A generic medicine which is not listed in Part VIIIA of the • Brand name or manufacturer/supplier; and
Drug Tariff. • Pack size — where there are multiple pack sizes available.
If the product does not have a price listed on the Dictionary of Medicines and Devices
(dm+d), also endorse:
• Net price before discount and exc. VAT
A branded medicine. • The only endorsement required is the pack size. If the product does not have a price
listed in the dm+d, the net price before discount and excluding VAT should also be
endorsed
A generic appliance which is only listed generically in Part • Pack size — where multiple pack sizes are available.
IX of the Drug Tariff. For elastic hosiery, prescriptions should indicate:
• Total quantity to be supplied;
• Description of the article (e.g. circular knit, below knee); and
• Compression class.
A generic appliance which is listed by brand or • Brand or manufacturer’s name as listed in the Tariff; and
manufacturer in Part IX of the Drug Tariff. • Pack size — only where there are multiple pack sizes listed in the Tariff.
Note: some appliances have default prices listed at the beginning of the relevant Drug Tariff
section, which are used for pricing in this situation. These are: catheters, elasticated tubular
bandages, absorbent perforated plastic film faced dressings, permeable non-woven synthetic
adhesive tape BP 1988, and peak flow meters. To be reimbursed for any of the above products
which may be priced higher than the default price, the prescription must be returned to the
prescriber for amendment to include a specific brand name as listed in the Drug Tariff.

An item listed in Part VIIID of the Drug Tariff XP to claim the £20 fixed fee to cover the costs incurred when sourcing the product.
And if required
BB – to claim broken bulk
Non- Drug Tariff Specials and imported unlicensed XP to claim the £20 fixed fee to cover the costs incurred when sourcing the product.
medicines Invoice price per pack size from which the product was supplied less any discount.
Pack size
MHRA manufacturers specials license/Wholesale dealer license number
Batch number if available.
The table indicates endorsing generics listed in Part VIIIA of the Drug
Tariff is completely redundant other than when more than one pack size
is listed. Most concession lines are still generics listed in the Part VIIIA
of the DT (all be it they have a temporary price uplift) and the endorsing
rules remain. No endorsement necessary, other than as per the table.
