Signed orders and Requisitions from Lead Information Pharmacist
Healthcare Professionals
Signed orders and requisitions are formal requests for medicinal products
that are ordered and supplied for use in practices/clinics. It is a handy
way for practices to get the medicinal products they need without a
prescription for a specific named patient.
Here is a guide to help pharmacies supply medicines via a signed order or
requisition.
Check it is written on the correct form:
• No designated forms are required for non-controlled drugs
signed orders or requisitions but they should be written on headed Information Pharmacist &
practice paper Medication Safety Officer
• For supplies of schedule 2 and 3 controlled drugs (CDs), they need
to be on the mandatory forms depending on where you are in the UK.
• England FP10CDF
• Wales WP10CDF
• Scotland
• Private supplies CDRF
• NHS supplies GP10A
• Northern Ireland
• Private Supplies CDRF1
• Health service supplies HS21
Check validity
• Signed orders/requisitions for schedules 2-4 controlled drugs are
valid for 28 days. Drug Tariff
• You may need a Wholesale Dealing Authorisation licence to Information Advisor
supply medicines via a requisition unless the following four criteria
can be fulfilled:
a) It’s an occasional supply
b) The quantity of medicines supplied is small
c) The supply is made on a not-for-profit basis
d) It’s not for onward wholesale distribution
