d. Dispose your waste / trash in the bin
e. All students should wear decent clothing inside and outside the dormitory.
i. Student must wear T-shirts and pants
ii. Provocative dresses are not permitted
f. Students should go home directly to the dormitory after the duty or the activity.
g. Littering and Loitering in the dorm specially when other students are sleeping or studying
is prohibited
h. Male students are not allowed to enter the room of the female students at all times and
vice versa
i. Students must submit a list of names of people who can fetch them during their vacant
periods which was duly authorized by their parents or legal guardians. An Identification
Card shall be presented when the person fetches the student and plans to go out of the
dormitory.
j. Visitors are only allowed in the reception area during the visiting hours set by the faculty
in charge.
k. Anyone leaving the dormitory premises should ask permission from faculty in charge.
l. Laundry and Hanging of wet clothes and towels is allowed in the designated areas only.
m. Students are expected to wash their own dishes and clean their litters when they leave
any place in the dorm
n. Napkins, tissues and panty liners are disposed in the appropriate trash cans only.
Flushing of these items in the bowls is strictly prohibited
o. Students doing scandalous and promiscuous acts shall be sent home and referred to
the Guidance Office for proper action.
p. Students should comply with other policies set by the dorm management and faculty in
charge
q. Incident Reports should be written by the concerned student immediately and submit to
the faculty in charge
r. Students sent home because of violating dorm policies will not be given a make-up duty.
s. The following habits are STRICTLY PROHIBITED:
Smoking/Vaping
Bringing and drinking alcoholic beverages
Bringing in of prohibited drugs, pornographic materials, and dangerous stuff
Immoral acts including public display of affection
Vandalism
Gambling
Procedures for Make Up Duty
1. A student must present to the Clinical Coordinator the following documents in order to
secure a make-up slip:
a. For demerit/absences: a letter of justification signed by the absentee and
addressed to the clinical instructor concerned, as well as an admission slip duly
approved by the clinical coordinator and the dean. Letter of excuse must be sent
with necessary attachments. The Clinical Coordinator determines the number of
hours needed for make up.
2. The Clinical Coordinator will prepare a schedule for the make up duty.
3. The student must secure make-up slip duly signed by the clinical coordinator and the
dean.
