a.  Clinical area attendance is a must. Students should complete the required RLE
                                       hours.  A failing mark will be given to the student who missed more than 20% of
                                       the allotted RLE hours.

                                    b.  Students should come on time. Tardiness is discouraged.

                                    c.  Students who arrive later than 16 minutes after the scheduled call is marked
                                       absent.

                                    d.  Attendance to case presentation is part of the clinical duty hours.


                       2.  Guidelines for make-up duties:

                       Tardiness:
                          1.  A student is considered late when he/she arrives 1-15 minutes after the scheduled time.
                          2.  Tardiness of more than 15 minutes after the scheduled time is equivalent to 1 unexcused
                              absence corresponding to 3 days make-up duties.
                          3.  3 recorded tardiness or 1 excused absence is equivalent to 1 make up duty.

                       Absences:
                       One (1) unexcused absence-------------3 days make-up duties
                       One (1) excused absence-----------------1 day make-up duty

                          4.  The students will shoulder the cost of each make-up duty.

                        3.  Hospital Uniform
                            1.  Students must wear the prescribed nursing uniform. Uniform should be clean and well
                                pressed.
                            2.  Students must wear their nameplates all the time.
                            3.  Level III and IV students who have received their caps must wear their caps and pins while
                                on duty.

                       4. Community Uniform
                            1.  Student must wear their prescribed community uniform.
                            2.  Students must wear their nameplate all the time.

                       5. Operating Rooms/ Delivery Rooms Uniform

                              1.  Students must go to the hospital in their duty uniforms. They only wear their OR/DR
                                 uniform inside the OR/DR, which consists of OR/DR suit with camisole for females and
                                 males, cap, mask, and rubber slippers.
                              2.  When students go out of the OR/DR, they must change their clinical uniform over the
                                 OR/DR suit and change their slippers, or wear their prescribed smock gown.

                            3.  All students must follow the guidelines for immersion:
                              A.  Curfew Hours:
                                   i.   All students must be in their respective boarding houses at 9:00 p.m.
                                   ii.   All lights must be put off at 11:30 p.m.; however this could be altered on a case-
                                        to –case basis.
                             B.   Visiting time:
                                   i.   Visiting time is up to 9:00 p.m.
