Giving feedback
Receiving feedback from families is very important to us. You can tell
us what you thought of your recent experience and help us to improve
our services. To complete an online survey please visit
www.kch.nhs.uk/about/how-are-we-doing. Our End of Life Care
survey can be found under ‘Care of the dying patient’. You will also
be given the opportunity to do this with the bereavement team, either
online or by completing a questionnaire to be sent back by post.
Obtaining a copy of medical records
If you would like a copy of the medical records, please contact the
Patient Advice and Liaison Service (PALS) or ask the bereavement
team. You can also download the Access to Medical records form with
the accompanying guidance notes to access to health records from
www.kch.nhs.uk. You can speak to a member of the Medical
Records team on 020 3299 3559.
How to raise a concern
Please ask a member of the bereavement team if you wish to have
an opportunity to discuss the cause of death, or raise any concerns
about the care and treatment. The team responsible for care would
usually be able to respond to your questions and concerns. An
informal meeting can be arranged in the bereavement office at a
mutually convenient time. There is no time limit for arrangement of
such a meeting, but please ask as soon as you feel ready to do so.
