We hope this leaflet will help you understand what you can expect
from King’s College Hospital. This leaflet has been divided into two
parts.

Part 1- Practical Information. This part guides you through some of
the practical procedures following the death of a friend or relative in
hospital. This includes registering a death and arranging a funeral.

Part 2 - Information on raising concerns and death reviews. This
part provides information about how deaths are reviewed by the Trust,
and safety and incident investigations that may be carried out. It also
explains how you may give us feedback, raise a concern or seek
independent advice.

Please do not hesitate to ask questions if you are unsure about
anything. Our staff will always be pleased to help.
