Death of a child
If your child has died in the hospital, the procedures outlined in this
booklet will apply. There is a specialist nurse available to support you
further.

We aim to guide you during this difficult time, and support you to make
choices and decisions that are right for you and your child. You will
be offered a ‘Memory Box’ and ceramic hand prints. These can be
given to you or kept until you are ready to collect them.
Government legislation requires all local authorities to review the
death of every child. The Child Death Overview Panel (CDOP) of the
borough in which you live will carry out this function. This is to see if
there is anything that can be done to improve services for children and
their families in the future. Please ask the bereavement team for

Death in the Emergency Department
All deaths which occur in the Emergency Department are reported to
the coroner. This is done the next working day after the death by the
bereavement team. The coroner’s officer will contact you directly to
inform you if a post-mortem examination is needed to establish the
cause of death, or if an MCCD can be issued either by the hospital or
the General Practitioner.
