At the point of registration, Lambeth register office also offers the ‘Tell
Us Once’ service. This allows you to report a death to most
government organisations in one go, such as the Passport Office,
DVLA, local council and the Department for Work and Pensions.
Please visit www.gov.uk for more information.

The registrar will give you the following:

• A certificate for burial or cremation
(known as the ‘green form’).
• You may also purchase copies of the death certificate at a
cost of £11 per copy. You will need a death certificate for
the will, any insurance claims, and for banks/building
societies. The register office does not accept cash payment
for death certificates.
