time of accrual, but in no event will be paid more than two (2) times the employee’s
                 yearly PTO allowance.


                 PTO Scheduling

                 Certain positions require a 30-day advance request for PTO scheduling.  Check with
                 your supervisor to determine what PTO scheduling is required for your position.
                 Requests for incidental PTO (day of request) should be directed to a supervisor for day
                 of request approvals.  Requests which are left on a voicemail are not “approved” but
                 are subject to supervisory approval.  Requests will be reviewed and evaluated based
                 on a number of factors including:

                     •  Business needs
                     •  Staffing requirements
                     •  Availability of PTO
                 Employees will not be allowed to have a negative PTO balance at anytime. If an
                 employee takes more PTO time than his/her yearly allowance, the employee will not be
                 paid for the difference.  An employee who uses all of his/her allotted PTO for the year
                 and wishes to take off more time before the end of the year may request approval for
                 a personal, unpaid leave of absence (see Leaves of Absence).  All PTO must be pre-
                 approved.  PTO approval is subject to the discretion of Jones-Hamilton Co.

                 PTO Usage

                 Paid Time Off (PTO) is paid at the employee's base rate of pay at the time of usage.  It
                 does not include overtime or any special forms of compensation such as incentives,
                 commissions, bonuses, or shift differentials.


                        Non –Exempt - Hourly
                        Non-exempt/hourly employees can take PTO in increments of no less than two
                        (2) hours, and no more than the number of regular hours worked in a single day
                        (8 hours, 12 hours).  The amount of PTO should be reflected on the employee’s
                        time card.
                        Example:
                            ▪  An employee works a 12-hour, three-day, workweek and takes 3 days off
                               in that workweek.  The employee would use 36 hours of Paid Time Off.
                            ▪  An employee works an 8-hour, five-day workweek, and takes 3 days off
                               within that workweek.  The employee uses 24 hours of Paid Time Off.
                        Exempt- Salaried

                        Salaried exempt employees can schedule for a partial day of PTO (4 hours), for
                        payroll purposes.  However, for attendance tracking, exempt employees should
                        document time away from the office by using the Absence Reporting Form
                        (ARF).
